<p><strong>Primary Care Navigator<br />

<p>Following a successful one-year pilot, run as a partnership between Age UK Kensington &amp; Chelsea, The Royal Borough of Kensington and Chelsea and NHS Kensington and Chelsea, we are now seeking a third navigator to join the team and continue gathering evidence of the project’s value to older people in the borough. This post will provide support to surgeries in the south of the borough.</p>
<p>The role is to provide time-limited guidance to people with long term conditions and to their carers. Key duties include establishing a plan with the GP, patient and carer; signposting the patient and carer to services available in the community – both free and where charges apply &#8211; and assisting their clients to access assessment for personal budgets.</p>
<p>As well as providing information to the patient and carer, and supporting them to access services, the Navigator will play a key role in informing commissioners about services, blocks and potential improvements.  This is part of developing our vision for excellent, responsive GP Commissioning which will push up the quality of local services.</p>
<p>The Navigator will be based in primary care, working from surgeries in the south of the Royal Borough.  Although based in the surgeries, the post holder will have a strong working relationship with Age UK Kensington &amp; Chelsea and will be part of the Information and Advice team.  They will also work with Age UK K&amp;C volunteers.  Supervision will be provided by Age UK K&amp;C with day-to-day management from the GP practice manager.</p>
<p>Successful applicants will have at least six months experience of working in health or social care, in direct contact with service users, in a paid or voluntary capacity. They will need to be excellent communicators and able to work effectively both one to one and as part of a team.</p>

<p>The HUB event is open to SUN Members and anyone eligible to use mental health services in Kensington and Chelsea. </p>
<p>There will be a number of speakers sharing their expertise by experience on a number of topics key to mental health service delivery.</p>
<p>Additionally, there will be a workshop on ‘Behavioural Activation’ facilitated by the Primary Care Psychological Service.  The aim of the workshop is to look at how we tend to stop doing the things that we enjoy or that bring us a sense of achievement when we&#8217;re feeling low, and the impact that this can have on us.  At the end of the session everyone should understand the importance of a balanced routine, and basic activity planning techniques.</p>

<p>The course is open to anyone living in Kensington &amp; Chelsea, Hammersmith &amp; Fulham and Westminister and has long term health condition. The course aims to help you take more control of your health by learning new skills to manage your condition better on a daily basis.</p>
<p> <strong>Who is the course for?</strong></p>
<p>Anyone living with any long term health condition such as: </p>
<p>-          Heart Disease<br />
-          High Blood Pressure<br />
-          Diabetes<br />
-          Breathing difficulties e.g asthma<br />
-          Depression<br />
-          Arthritis<br />
-          Cancer</p>
<p>Or any other long term health condition</p>
<p>The Free six week course covers:</p>
<p>- Healthy lifestyle advice including diet and exercise.<br />
- Managing pain, fatigue and stress, and relaxation techniques.<br />
-  Better communication with family, friends and health professionals.</p>

			<content:encoded><![CDATA[<p>Local people are invited to have their say on community health services and get more involved in how they are run – with the launch of Central London Community Healthcare NHS Trust’s public consultation.</p>
<p>CLCH, which runs community health services in Barnet, Hammersmith and Fulham, Kensington and Chelsea, and Westminster, wants to become an NHS Foundation Trust. As part of this process we are running a 12-week public consultation on our plans to improve integration across health and social care, make our services more patient-centred and change the way that we are run. Alongside this we are launching a campaign to recruit 10,000 local people to be members and get more involved in our organisation.</p>
<p>CLCH’s Chief Executive James Reilly said: “We are very excited about the opportunities that becoming a Foundation Trust will bring. By having a membership made up of local people, patients and employees who can elect Governors to work actively with us on their behalf, our local communities will have a real say in the way that their healthcare needs are met. We will also have greater local control over our finances. This will allow us to invest money gained through sound financial management to improve existing services and develop new ones.”</p>
<p>The public consultation starts today (08 May) and patients, staff and partners will be able to give their views until 31 July. We will then use the feedback we receive to further develop our business plan, which is an important part of our Foundation Trust application. We hope to become a Foundation Trust during the summer of 2013.</p>
<p>During the consultation period we will be holding a number of public meetings where we will explain our plans for the future and ask people to comment on them. We are also keen to arrange consultation meetings with community groups across the four boroughs we serve. The consultation document is also available from our website where it can be downloaded and returned by FREEPOST or completed online.</p>
<p>CLCH’s Chair Anne Barnard said: “We would like as many people as possible to respond to our consultation. We provide care for almost one million people who live and work in Barnet, Hammersmith and Fulham, Kensington and Chelsea and Westminster. It’s fair to say that if you live in one of the boroughs we serve you are likely to receive care from us at some point in your life, so it is important that we provide everyone in our communities with the opportunity to have their views on our plans heard.”</p>
